LPN - Crisis Care Every Weekend 8p-8a
Education: Current LPN license from State of Ohio
Licensure: Current valid driver's license, and must have own transportation.
Experience: This position is responsible for identifying and coordinating patient/family care needs to provide palliative and supportive care to the terminally ill and their families. He/She utilizes assessment and intervention skills to maximize quality of life to patient and family. The hospice nurse is committed to the philosophy, goals, and objectives of the hospice concept.
Excellent nursing process skills; well-developed communication skills; basic knowledge of family systems/dynamics, flexibility. 3-5 yrs prior medical/surgical nursing experience or equivalent; home health or hospice experience preferred.
Home Health Aide - STNA, Part-Time Weekends, Part-Time Weekdays
Education: Successful completion of nurse aide course that meets federal/state requirements.
Licensure: Current state tested nursing assistant, current valid driver's license, and must have own transportation.
Experience: 1-2 yrs experience providing patient care in home health, hospice, acute care, ECF or ALF required. Hospice or Home Health experience preferred.
RN - PRN Days and PRN On-Call
Education: Graduate of approved school of nursing.
Licensure: Current RN license from State of Ohio, current valid driver's license, and must have own transportation.
Experience: Excellent nursing process skills; well-developed communication skills; basic knowledge of family systems/dynamics, flexibility. 3-5 yrs prior medical/surgical nursing experience or equivalent; home health or hospice experience preferred.
