Your Privacy

Hospice of Miami County is required by law to maintain the privacy of your health information and provide to you and your representative a written notice of our duties and privacy practices. Hospice of Miami County is committed to protecting the privacy of our patients and has established a policy to protect against unnecessary disclosure of patient health information.
As a hospice patient, we believe you have the right to:
- Receive a paper copy of the Notice of Privacy that describes how medical information may be used and disclosed and how you can access this information.
- Voice complaints about our privacy practices to the Secretary of Health and Human Services or by contacting the Hospice Privacy Officer at 937-335-5191.
- Request restrictions on the uses and disclosures of your health information.
- Request to receive confidential communications.
- Access your protected health information for inspection and/or copying.
- Amend you health information.
- Request an accounting of disclosures and health information.
